Please find attached the Federal Cash Request Form, Instructions and the Assurance Statement.
The Federal Cash Request process allows email transmission of the Federal Cash Requests forms. As a part of this process, the district superintendents should sign a Statement of Assurance, identifying the individuals in their district office who are authorized to prepare and submit these forms electronically. This Statement of Assurance is the superintendent's blanket approval of these documents.
We are requesting that the Federal Cash Request Statement of Assurance forms be updated for the new fiscal year beginning July 1, 2008. Attached is the blank form to be completed. Please fill in the school district name and the names, position titles, and email addresses of individuals who will be preparing and submitting Federal Cash Requests. The completed Statement of Assurance cannot be sent by email to Frankfort. The Statement of Assurance must be printed, signed by the Superintendent and mailed to Frankfort at the following address:

Please begin to use the attached Federal Cash Request workbook for all requests submitted after July 1, 2008. Your current Federal Cash Request workbook should be discarded.
