All students that drop out of school must complete the Student Dropout Questionnaire documenting their reason for leaving school. School guidance counselors will complete this questionnaire at the one (1) hour counseling session with students and parents prior to dropping out. KDE does not collect and retain copies of this document, but the questionnaire should be retained in the student's permanent file.
