Welcome to the Transition Toolkit, a “one stop shopping” source that schools can access as they develop, implement or revise their plans for transitioning students from the elementary grades to the middle grades. The activities included can be implemented with both students and parents during the year prior to the transition and continuing throughout the first transitional year.

As a virtual resource, the toolkit will be expanded to include the transition to high school and the unique needs of students in alternative settings. As you explore the toolkit, please share your thoughts and experiences so that we can add or revise activities. We have featured the work of several Kentucky school districts and would like to include samples from your district as well.
