The presentation includes guiding questions for secondary transition planning for youth with significant disabilities, as well as student scenarios.

The Individual with Severe Disabilities (Case Study).pdf from the National Secondary Transition Technical Assistance Center (NSTTAC) provides examples and non-examples of postsecondary goals, annual goals, transition services, evidence of coordination, age-appropriate transition assessment and course of study for a student with severe disabilities.
